Caregiver and Care Recipient must:

1. Be at least 18

2. Live in the same household                and not maintain another                    residence

3. Be willing to undergo a criminal          history search (caregiver only)

 

Care Recipient must:

 

1. Require assistance with at least          three activities of daily living such      as help with eating, dressing,            toileting, etc., or have other                substantial medical condition

2. Meet other Indiana Medicaid              Waiver eligibility requirements
